New Client Intake Paperwork

Please download and complete all required intake paperwork before your first appointment. Completed forms must be returned at least 48 hours before your scheduled appointment, along with a copy of your driver’s license or other government-issued photo ID and copies of the front and back of your insurance card. You may return these documents by email, fax, or mail using the contact information provided. Please allow sufficient time for mailed documents to arrive by the 48-hour deadline.
If we do not receive all completed intake paperwork and required documents at least 48 hours before your scheduled appointment, we will need to reschedule your appointment. Receiving these materials in advance gives your provider adequate time to review your information and, when applicable, obtain insurance benefit information before your appointment.

MindWell Psychology Group providers will make every reasonable effort to contact your insurance company and obtain information regarding your eligibility and benefits. However, understanding and verifying your insurance coverage is ultimately the client’s responsibility. Information provided by an insurance company is not a guarantee of coverage or payment. Clients are encouraged to contact their insurance carrier directly before their first appointment to confirm behavioral/mental health benefits, deductibles, copayments or coinsurance, authorization requirements, network status, and any other limitations or requirements that may affect coverage. The client remains responsible for charges not covered or paid by their insurance plan, consistent with the practice’s financial policies, and should update their provider if their insurance changes.
Private Pay — $125 per hour
Clients may use private pay when they do not have insurance, their insurance does not cover a service or provider, or, when permitted by applicable law and the provider’s insurance agreements, they choose not to use available insurance benefits. Some clients elect private pay for greater privacy and control over information disclosed to their health insurance plan, as billing insurance generally requires the disclosure of certain clinical and diagnostic information. Private-pay clients are responsible for the full cost of services under MindWell Psychology Group’s financial policies. Upon request, we may provide appropriate documentation for clients who wish to seek reimbursement independently; however, reimbursement is not guaranteed. Clients should contact their insurance carrier regarding coverage, reimbursement, or restrictions related to choosing not to use their insurance benefits.
